The Feedback activity is a survey tool that allows for custom questions and non-graded questions.

  • On your course page, toggle the Edit mode button on in the top right of the screen to turn editing on.

  • In the section of your page that you want to add the feedback, go to the bottom of the section and click on+ Add an activity or resource or select the plus button between activities and resources.

  • Select Feedback from the Activities tab.

  • Update the name and description and desired settings on the settings screen.

  • Ensure to fill out the fields in the Question and submission settings and the After submission setting. Record user names is set to Anonymous by default.

  • Once complete, click on Save and display on the base of the screen.

  • Once your settings have been set Save and display. This will take you to the next page where you can Edit questions  by clicking on the Edit questions button.

  • Add questions by clicking on Save as new template or Import questions from the Actions dropdown menu.

  • Or alternatively, add questions manually by selecting the relevant option from the Add questions dropdown.
